            1. TART BY ENROLLING WITH CBS
            Complete and sign the CBS Enrollment Forms.
            If not enrolling through our online enrollment process please return the completed
            forms to receive your Collection Kit. If you fax these forms, please make sure the
            information is legible. If you chose payment option #1 and are paying by check please

            2. CCEPT RECEIPT OF YOUR COLLECTION KIT
            Upon enrollment confirmation, your Collection Kit will immediately be sent to you
            by FEDEX ground, UPS or U.S. Postal Priority Mail. This service is included in your
            fee.
            
            For parents ordering the Kit within 30 days of the expected due date, Expedited
            Service may be necessary with an additional charge.
            
            3. ERIFY THAT YOUR HEALTHCARE PROVIDER KNOWS YOU
                HAVE CHOSEN CBS
            Discuss with your physician/healthcare provider at your next visit that you have
            chosen to collect the umbilical cord blood using CBS. Let them know we will be contacting
            them with instructions and/or questions regarding our process of collection.
            
            4. XPECTING YOUR DELIVERY
            Remember to take your cord blood collection kit with you to your birthing center
            when you are ready to deliver. We recommend packing it along with your other personal
            items in the overnight bag you have "ready to go" when you are close to your anticipated
            due date. Store the Kit at room temperature at all times to avoid exposure to temperature
            extremes. Do not leave the Kit in your car.
            
            5. OLLECTION OF SAMPLE
            Immediately after your baby’s delivery, the umbilical cord is clamped and the baby
            separated from the cord. At this point, the needle attached to the collection bag
            is inserted into the vein in the umbilical cord. The placental blood/umbilical cord
            blood, drawn by gravity, then flows into the collection bag. The bag comes equipped
            with an anti-coagulant to keep the blood from clotting before it reaches our laboratory.
            The process is non-invasive, painless and generally takes between three and five
            minutes to complete.
            
            6. E SURE TO CONTACT CBS OR PREARRANGED COMPANY
                FOR TRANSPORT
            Once this simple collection process is complete, call CBS to arrange for the Courier
            to Pick-Up and deliver the sample to our contracted AABB certified, federally regulated
            lab and storage facility in Paramus, NJ. The kit is specifically designed to meet
            all regulatory requirements for shipping blood and to ensure that the package is
            insulated and padded to protect the sample from any damage. Our courier guarantees
            temperature control for the entire delivery process. We are available 24 hours a
            day, 365 days a year. The kit should be sealed and remain with you until the courier
            arrives. Note, the medical courier will come directly to your hospital room. Please

            7. TARTING TIME FOR PROCESSING CORD BLOOD AND
                NOTIFICATION WHEN COMPLETED
            Once everything is received at the lab, tests are performed on the cord blood sample
            to determine volume, nucleated cell count, and sterilization. Infectious disease
            testing is performed on the mother's blood. Once tested, the cord blood unit is
            cryopreserved in cryoprotected blood bags at -196 degrees Celsius (-321degrees Fahrenheit).
            We store in blood bags because studies have shown that bags allow for the best cell
            recovery rates after freezing, thawing and washing, and an even higher cell viability
            rate than other storage containers such as cryovials. The cryopreservation process
            includes submersion of your sample in liquid nitrogen at all times because this
            has proven to be the most effective method. After the storage process is complete,
            you will receive notification in approximately 10-12 weeks for confirmation
            of sample viability.
